Assessing the right health and social care services can often be a complex and time-consuming process. PA Navigate recognized this challenge and offers a secure and user-friendly platform designed to simplify how Pennsylvanians connect with essential community resources.
Managed by four state-certified health information organizations, these groups collaborate to ensure seamless service delivery. This innovative project was supported by funding from the Pennsylvania Department of Human Services.
